Initial 175 experience in the use of cyanoacrylate occlusion for the treatment of incompetent great, small and accessory saphenous veins

Purpose : Cyanoacrylate closure of the great saphenous vein with the VenaSeal System is a relatively new modality. The objective of this study was to report the initial early results of cyanoacrylate closure for the treatment of incompetent great saphenous veins(GSV), small saphenous veins(SSV), and/or accessory saphenous veins(ASV).
Methods : There were 175 patients (292 legs; 363 truncal veins) who underwent cyanoacrylate closure between 2019 and 2021 in single center. 150 patients(85.7%) received concurrent treatment using foam sclerotherapy. Intraoperative tumescent anesthesia or post-procedure compression stocking were not used. 171 patients were treated with mild sedatives with monitored assist care. All patients took antihistamine for 2 weeks after discharge. Patients revisited the clinic on 10 days, 1 month, and 3 months after surgery. Duplex ultrasound was performed at each visit.
Results : In 2 patients, cyanoacrylate was injected into the deep vein, but it was resolved immediately with intervention. All treated veins (256 GSVs and 94 SSVs and 13 ASVs) had complete closure (100%) by duplex ultrasound at seven days and one month. 2 recanalization was occurred 3months later.
Transient superficial phlebitis was reported in 22 of 175(12.5%) patients, which was self-limited. 144/175(82.2%) patients were “completely” or “somewhat” satisfied
Conclusion : Cyanoacrylate closure is safe and effective for the treatment of one or more incompetent saphenous or accessory saphenous veins. Closure rates were high even in the absence of the use of compression stockings. Patients are highly satisfied and report low postoperative complications.
