Predictive factors for improvement of left ventricular function after coronary bypass surgery in ischemic cardiomyopathy

Purpose : Coronary artery occlusive disease (CAOD) is a main cause of ischemic cardiomyopathy (ICMP). Coronary artery bypass grafting (CABG) is a treatment choice of multivessel coronary artery occlusive disease and ischemic cardiomyopathy. Our study is an emphasis on predictive factors for improvent of left ventricular function by analysis of pre and intraoperative data.
Methods : Between January 2008 and December 2015, 1,873 patients underwent coronary artery surgery with or without other cardiac procedures. Of these, 216 patients has a low ejection fraction (EF < 35%), and underwent isolated CABG or off-pump coronary artery bypass grafting (OPCAB). Pre and post-operative echocardiographic assessments were collected and analyzed. In addition to echocardiographic data, we estimated pre-operative and intra-operative factors. Improvement of LV function is defined as increased left ventricular ejection fraction (LVEF) after surgery which calculated ratio of pre and post operative EF.
Results : In the present study, 124 (57%) patients had > 0.1 (10%) increased LVEF (group A) and 92 (42%) patients had < 0.1 (10%) increased LVEF or decreased (group B). Patients who had high NYHA score (p=0.04) or pre-operative intra aortic balloon pump (p=0.02) belongs to a group A. Pre-chocardiographic findings, low LVEF (p=0.01) and high LV end diastolic diameter (p=0.03), are similarly checked in group A.
Conclusion : In ischemic cardiomyopathy patients, LVEF is a parameter of LV function and easily measured by echocardiography. LV function improvement after surgery can be expected for patients with high pre-op NYHA score and IABP. Echocardiographic assessement is a major source of finding predictive factors.
