Carotid endarterectomy versus carotid artery stenting in elective surgery: A 9-year single institutional analysis

Purpose : About 20% of ischemic strokes are extracranial cerebrovascular disease. Carotid endarterectomy (CEA) was the gold standard for carotid artery stenosis treatment until carotid artery stenting (CAS) was introduced in the 1980s. Since then, there have been many multicenter, randomized trials, but it is still not clear which of the two methods is superior. We tried to confirm the effectiveness and safety of CEA and CAS through the experience of a single center.
Methods : This study is a single center, retrospective study, and we analyzed patients who underwent CAS and CEA as elective surgery from January 2012 to December 2020. We investigated the baseline characteristics, postoperative complications, and outcomes of both groups.
Results : Of the total 235 patients, 128 underwent CAS and 107 underwent CEA. Within 30 days postoperatively, myocardiac infarction was 1 (0.8%) in CAS and 1 (0.9%) in CEA, and there was no statistically significant difference (p = 0.899). Within 30 days postoperatively, cerebral infarction was 4 (3.1%) in CAS and 1 (0.9%) in CEA, and there was no statistically significant difference (p = 0.247). Within 30 days postoperatively, death was 1 (0.4%) in CAS and 0 (0.0%) in CEA, and there was no statistically significant difference (p = 0.360).
Conclusion : In elective surgery, CAS and CEA had the same effect of preventing cerebral infarction, and there was no difference in post-operative complications.
