Does position of prosthetic valve affect a long-term durability of prosthetic valve in pulmonary position?: Focusing on the angle between prosthetic valve and main pulmonary artery

Purpose : The objectives of this study were to evaluate the long-term durability of prosthetic valve, focusing on the angle between prosthetic valve and main pulmonary artery.
Methods : Between January 2001 and May 2020, 58 consecutive patients with TOF underwent PVR after total repair. Of them, 50 patients underwent redo PVR due to prosthetic valve failure. We finally reviewed 36 patients, excluding those who did not have adequate imaging results. Only 6 were female patients. Mean age at total repair of TOF was 19.8±20.7 months and mean age at PVR was 10.7±5.7 years.
Results : The follow-up completeness was 94.4% and mean follow-up duration was 15.6±2.5 years after first PVR. During follow-up, there was 2 operative mortality and 5 major complications. Follow-up echocardiographic examination demonstrated significant improvement in both TR (p = 0.03) and PR (p < 0.01) after PVR. Right ventricular end diastolic volume index decreased from an average 155.3±45.4 to 120.3±21.9 ml/m2 (p < 0.01) after redo PVR on MRI data. Regarding the relationship between the angle and prosthetic valve failure, the duration taken until the occurrence of valve insufficiency is significantly shortened when implanted at an angle of 45 degrees or more.
Conclusion : Same as previous literature, long-term outcome of PVR results in significant clinical improvement and lasting recovery in ventricular size and function. In addition, the higher the angle, the faster the valve insufficiency is induced. We would like to present a new perspective on whether the angle of valve implantation affects prosthetic valve failure.
