Outcomes of surgical treatment for Non-small cell lung cancer in high-risk patients

Purpose : Surgery is the mainstay of the treatment for Non-small cell lung cancer (NSCLC). Patients with compromised lung function are considered high risk, and they may be denied surgical treatment. This study aimed to investigate outcomes and prognostic factors for high-risk patients who received surgical treatment.

Methods : Between January 2012 and December 2017, 471 patients who had curative lung resection for primary lung cancer reviewed retrospectively. Definition for the high-risk patients followed ACSOGZ/RTOG 1021 criteria.

Results : Among the 471 patients, 77 of high-risk(HR) and 394 of standard-risk(SR) patient group. HR group had more postoperative complications more than grade 2. (39 patients (50.6%) in HR vs. 84 patients (21.3%) in SR, p<0.001). HR group also showed poorer overall survival (p<0.001). On multivariate analysis, lobectomy, adenocarcinoma, earlier stage were significant prognostic factors but not HR (Table 1). HR patients without postoperative complications showed a comparable overall survival with SR patients (Figure 1).

Conclusion : Survival after surgery in HR patients were more affected by postoperative complications. Careful patient selection and care to reduce postoperative complications are more critical in HR patients.
