One and Half Repair of Ebstein’s Anomaly with End Stage Heart Failure and Liver Cirrhosis

Purpose : We reported the case of Ebstein’s anomaly with end stage heart failure and liver cirrhosis treated with one and half repair.
Methods : A fifty-seven-year-old female patient visited the emergency room due to severe dyspnea. She was diagnosed Ebstein’s anomaly 23 years ago and lost follow-up. She had severe jaundice and swelling in her leg. On laboratory findings, the total bilirubin was 25 mg/dl and the platelet count was 66,000. On chest X-ray, very severe cardiomegaly was observed. On echocardiography, ejection fraction rate was 40%, and atrialized right ventricle with severe tricuspid regurgitation(TR) were observed.
Results : She underwent one and half repair: Tricuspid valve replacement with tissue valve, atrilized right ventricle plication, bidirectional cavopulmonary shunt with ligation of azygous vein. Extracorporeal membrane oxygenation(ECMO) was applied with venting of pulmonary artery due to failure of weaning off from cardiopulmonary bypass(CPB). A sternum was left open. Postoperatively, she underwent delayed sternal closure on 6th postoperative day. ECMO was weaned off successfully on 9th postoperative day. She was discharged on 45th postoperative day and had mild dyspnea.
Conclusion : One and half repair is an effective surgical option for Ebstein’s anomaly with severe right ventricular dysfunction.
