The impact of prophylactic tricuspid annuloplasty in concomitant bioprosthetic aortic valve replacement.

Purpose : In mitral valve surgery, concomitant tricuspid annuloplasty (TAP) in moderate tricuspid regurgitation (TR) improves long-term right-sided remodeling. We tried to define the impact of prophylactic tricuspid repair in aortic valve replacement (AVR).

Methods : This study is a single-center retrospective cohort study. From 1994 January to 2019 December, patients with mild to moderate TR and severe aortic stenosis who underwent aortic valve replacement with bioprosthesis were reviewed. Concomitant mitral valve surgery or mechanical AVR were excluded. The primary endpoint is major adverse cardiovascular event (MACE), composed of cardiac death, myocardial infarction, stroke, and major bleeding. The secondary endpoint is freedom from moderate to severe TR and recovery of right ventricular function.

Results : 120 patients received bioprosthetic AVR and 31 patients (25.8%) had concomitant tricuspid annuloplasty (TAP group). TAP group had large left atrial diameter (50.6mm vs 45.5mm, P=0.012), more patients with atrial fibrillation (54.8% vs. 22.5%, P=0.002) and right atrial enlargement (71% vs. 29.5%, P<0.001) than in non-TAP group(Table). TAP group had rate of freedom from MACE at 5 years (96.8% vs. 82.7%, P=0.207) with non-TAP group (Fig.4). Both groups also had similar rates of freedom from moderate to severe TR and recovery of right ventricular function at 5 years.

Conclusion : Although patients who had TAP had higher risk for MACE, the event rate was similar between two groups. TAP may be beneficial in patients who would have bioprosthetic AV with significant TR.
