Two-port Approach for Robotic Mediastinal Surgery Experienced In a Single Institution

Purpose : The introduction of robotic surgery for the mediastinal disease had added more benefits in the dissection of the mediastinum with a three-dimensional view and multi-articulated moving instruments. We report our experiences of two-port robotic mediastinal surgery experienced in the process of our continuing changing the surgical approach from conventional three or four-port and two-port to subxiphoid method.

Methods : Between May 2018 to January 2020, 10 patients underwent a two-part approach for robotic mediastina surgery. Thymoma (n=5), thymic cyst (n=3), and teratoma (n=2) were included in our case series. We approached the thoracic cavity from the right side in 4, left in 2, and subxiphoid in 4 patients. We reviewed the technical advantages/disadvantages of the two-port approach and the perioperative outcomes to define the role of the robotic surgery with a reduced port.

Results : We approached on the right thorax in 4 patients, left thorax in 2 patients, and subxiphoid in 4 patients who were the last series. The mean operation time was 175.5±70.7 minutes (ranges 130-363, median 145.5). We performed two-port robotic mediastinal surgery without conversion to multiport or open surgery, and the chest drain was removed at mean 3.9 days and discharged at mean 6.2 days. We experienced prolonged air leak in one case through the subxiphoid approach because of severe intrathoracic adhesion. There was no readmission case and delayed complications.

Conclusion : A two-port approach for robotic mediastinal surgery is feasible and safe for achieving a smooth transition from transthoracic multiport to uniportal subxiphoid method.
