Unexpected N2 LN metastasis in early lung adenocarcinoma without hilar/interlobar LN metastasis

Purpose : It has not been widely reported that how N2 LN metastasis (N2M), especially non-lobe specific LN metastasis (non-LSN2M), manifested in patients with early lung adenocarcinoma (ADC) not accompanying hilar/interlobar LN metastasis. The purpose of our study was to investigate the variables affecting N2M and non-LSN2M in those patients.
Methods : In our retrospective study, 256 patients who underwent lobectomy with sufficient mediastinal LN dissection for cN0M0 ADC with pathologic invasiveness size of 2cm < and ≤ 3cm, not accompanying hilar/interlobar LN metastasis were included in the study cohort. Based on the study cohort, the risk analysis for the N2M was conducted. By excluding the patients with RML cancer from the study cohort, the subgroup cohort was made. Based on the subgroup cohort, the risk analysis for the non-LSN2M was conducted.
Results : Of all, N2M rate was 8.2% (21/255). Among them, 33.3% (7/21) was non-LSN2M. In multivariate logistic regression analysis, the lymphatic invasion (LI), primary tumor maximum standardized uptake value ≥ 2.9 g/ml (high SUVmax), and visceral pleural invasion (VPI) predicted N2M well (P=.022, .007, and .043, respectively). Among them, VPI also predicted non-LSN2M well (P=0.012).
Conclusion : In patients with early ADC not accompanying hilar/interlobar LN metastasis, high SUVmax, LI, and VPI predicted N2M well. Furthermore, VPI also predicted non-LSN2M wall. Our results could help to define the adequate LN dissection strategy for patients with early ADC.
