Epidemiology and management of the first episode of primary spontaneous pneumothorax in juvenile and young adults: a nationwide study in Korea

Purpose : Most of previous studies have described epidemiology of spontaneous pneumothorax, regardless of type or number of episodes. The epidemiologic study focused on the first episode of primary spontaneous pneumothorax (FEPSP) is rare in young age population. The aim of this study was to describe the characteristics of epidemiology and management of FEPSP in juvenile and young adult.

Methods : The data from all patients treated in outpatient clinics or hospitalization with a diagnosis of pneumothorax in Korea from 2002 to 2017 were retrieved from the National Health Insurance Service database in Korea. A total of 74,630 patients aged from 10 to 39 years with FEPSP from 2005 to 2016 were enrolled.

Results : A 64,839 (86.9%) of patients were admitted and 9,791 (13.1%) were managed at outpatients department as initial treatment. The average annual incidence rate was estimated 29.3/100,000 person-year. The ratio of male to female incidence rate was 8.3. The incidence rate increased from 22.1/100,000 person-year in 2005 to 29.2/100,000 person-year in 2016 (p for trend = 0.016). The proportion of surgical treatment increased from 33.8% of inpatients in 2005 to 37.4% in 2016 (p for trend < 0.001). The proportion of thoracoscopic surgery increased from 88.8% of surgical patients in 2007 to 97.0% in 2016 (p for trend < 0.001). Within 1 week, 26.1% of outpatients required hospitalization.

Conclusion : There are increasing trends in annual incidence rate and proportion of surgical treatment in the FEPSP. In outpatient management, next follow-up visit should be considered within 1 week.
