Coronary Artery Bypass Grafting in Pediatric Patients for Kawasaki Disease

Purpose : Kawasaki disease in pediatric patients is the most common cause of coronary disease. It is the first indication for pediatric coronary artery bypass grafting. This study aimed to review the long-term clinical outcomes and graft patency of coronary artery bypass graft surgery.
Methods : Between March 2004 and March 2013, six patients with Kawasaki disease underwent coronary artery bypass grafting surgery. All patients were male with ages at operation ranging from 7 to 20 years (median, 13.0±5.1 years). The left internal mammary artery, the right internal mammary artery, and saphenous vein were used in 5 (83.3%), 1 (16.7%), and 2 (33.3%) cases, respectively. Saphenous vein were used as a composite graft in one case, and an aortocoronary bypass graft in the other case. 4 patients (66.7%) underwent single CABG, and 2 patients (33.3%) underwent multiple CABG.
Results : The mean follow-up duration was 9.7±4.7 years. All patients survived. There was no postoperative complication and operative mortality. One patient (16.7%) had symptoms such as exertional chest pain and dyspnea on exertion, and one patient (16.7%) had ventricular tachyarrhythmia. They improved in subjective symptoms after operation. In most patients, postoperative SPECT findings showed the improved perfusion, compared with preoperative SPECT. All Grafts were patent, even in saphenous vein grafts, as checked coronary angiography or computed tomography angiography.
Conclusion : Coronary artery bypass graft surgery is a good surgical option in pediatric patients with coronary lesions caused by Kawasaki disease, in terms of graft patency and myocardial perfusion.
