Long-term Result of Sandwich Technique for Multiple Ventricular Septal Defect Repair

Purpose : The management for multiple ventricular septal defects (VSDs) remains a surgical challenge. In the neonate and infant, primary repair can be demanding. This study aimed to review the long-term clinical outcomes in the single center.
Methods : Between June 2006 and April 2019, 14 patients with Multiple VSD underwent VSD closure using sandwich technique. 6 patients were male and 8 patients were female. There had associated anomaly such as ASD (n=13), CoA (n=1). Mean age at operation was 7.23±8.8 months (range, 4 days-27 months). 2 patients underwent previous sternotomy due to PA banding. Echocardiographic studies before and after the repair was evaluated.
Results : The mean follow-up duration was 4.2±3.7 years. Postoperative complications included low cardiac output in one, cardiac arrest in one, and Arrhythmia in one. There was postoperative mortality in 1 patient, causing cardiogenic shock. There was no late death. There were residual shunt postoperatively, but that were spontaneous closure during follow-up. Left ventricular ejection fraction(LVEF) were slightly decreased postoperatively, but that were improved ventricular function on latest echocardiography. Overall survival was 92.9% in 12 years.
Conclusion : Methods for the repair of multiple VSDs have evolved over the years, with improved outcomes owing to advancements in surgical and interventional techniques. However, the absence of specific guidelines for the treatment of this specific congenital heart lesion makes decision-making difficult. In our center, Sandwich technique appeared to be a good option for multiple VSDs in terms of clinical outcomes.
