Surgical Outcomes of uncrossing Procedure in Patients with Vascular Ring due to Circumflex Aorta

Purpose : The purpose of this study is to describe long-term result of surgical management of circumflex aorta and postoperative complications of the surgical procedure.

Methods : We reviewed 5 patients with circumflex aorta from january 2004 to march 2018. Uncrossing procedure was done in 4 patients and median follow-up period was 9.7 years

Results : Median CPB time was 128 minutes. Circulatory arrest with deep hypothermia and antegrade selective cerebral perfusion were used in 3 patients (75%). There was no in-hospital and long-term mortality but frequent reintubation (n=2, 50%) and tracheostomy (n=2, 50%) due to vocal cord palsy(n=3, 75%) and tracheomalacia(n=2, 50%) . All patients had their symptom improved after the surgery.

Conclusion : Aortic uncrossing procedure is safe surgical procedure with no in-hospital and long-term mortality. But respiratory complication is frequent because circumflex aorta is often associated with tracheomalacia and the aortic uncrossing procedure sometimes results in bilateral vocal cord palsy.
