Echocardiographic Parameter to Predict LV Systolic Functional Lmprovement after OPCAB for Severe LV Dysfunction

Purpose : Several imaging techniques are available for the assessment of viable myocardium, but left ventricular (LV) functional improvement following coronary artery bypass grafting (CABG) is difficult to predict.

Methods : 133 patients with severe LV dysfunction (ejection fraction[EF]≤35%) who underwent off-pump CABG between January 2013 and December 2017 were enrolled. All patients were examined by transthoracic echocardiography preoperatively, before discharge, and at 3 months and 1 year after CABG.

Results : Improvement of LV EF was observed in 102 patients (76.7%); 55 patients (41.4%) before discharge, 38 patients (28.6%) at 3 months after CABG and 9 patients (6.8%) at 1 year after CABG. Preoperative diagnosis and incomplete revascularization were not the predictors of improved LV function after CABG. The only independent predictor for functional recovery after CABG is the number of thinned segment of LV. (p=0.027)

Conclusion : Global LVEF improved three quarters of patients, mostly during the first 3 months after CABG. And LV dysfunction is likely to recover in patients who had less thinned segments of LV.
