Diagnostic Accuracy of PET/CT for Detecting the Metastatic Lymph Nodes in Esophageal Squamous Cell Carcinoma

Purpose : This study was performed to evaluate the value of PET/CT to detect the metastatic locoregional LNs in esophageal squamous cell carcinoma.

Methods : Between 2013 and 2019, 124 patients who received the upfront esophagectomy with total mediastinal LN dissection, three-field LN dissection without neoadjuvant therapy after preoperative PET/CT and EUS were retrospectively reviewed. The LN stations were defined based on the 11th Japanese classifications. The sensitivity, specificity, positive and negative predictive values were evaluated in each LN stations. When the FDG uptake of regional lymph nodes was visually greater than the surrounding tissue.

Results : The mean age of patients was 63.23±7.46 years-old with 107(86.2%) male patients. The mean numbers of dissected LNs were 71.58±24.99. The numbers of clinically suspected and pathologically confirmed LN stations were 0.61±1.153 and 1.25±1.82, respectively. In only 61(49.1%) patients, the clinically suspected LN stations were correlated to the pathologic LN station. In thorax, bilateral recurrent laryngeal LNs showed low sensitivity (34.615% in right and 35.714% in left), whereas the other LN stations such as paraesophageal and left tracheobronchial LNs showed sensitivity less than 20%. In abdomen, the left gastric LNs showed relatively high sensitivity (52.381%) whereas celiac and common hepatic LNs showed poor sensitivity. In neck area, bilateral cervical paraesophageal LNs showed poor sensitivity whereas right supraclavicular LNs showed high sensitivity.

Conclusion : In ESCC, detection rate of metastatic lymph nodes by PET/CT was low and the sensitivity and specificity of each LN stations were different.
