Expandability of Cephalic Veins after Brachial Plexus Block in Arteriovenous Fistula Formation for Hemodialysis

Purpose : Arteriovenous fistula(AVF) for hemodialysis is essential for end-stage-renal-disease patients. The choice of vessels is important in AVF formation. In previous studies of the authors, more than 3mm diameter was presented as one of the criteria for choosing autologous vein and is applied to surgery. However, if they did not meet the venous diameter standards, the veins showed satisfactory enlargement after brachial plexus block(BPB). Our study’s purpose is to find out how far BPB could expand veins, and the surgical outcomes of using the expanded vein after BPB.

Methods : Total of 61 patients were examined from August 2018 to December 2019 who received AVF formation. Clinical characteristics, hemodynamic indicators including vascular diameters, and complications were investigated. Patients with a vein diameter of less then 3mm before BPB were classified into A group (26people) and B group (35people) for patients with a vein diameter of 3mm or more.

Results : The post-anesthesia expandability of the entire patient was 41%, 62% in Group A and 25% in Group B with significant difference(p=0.001). The average of blood flow and pulsatility index showed no significant difference between Group A and B. The incidence of complications also showed no significant difference.

Conclusion : Sufficient venous expandability was shown after BPB. the results of AVF formation performed with expanded veins were no different. Therefore, even if the venous diameter before anesthesia is less than 3mm and has been sufficiently expanded after BPB, the surgery can be carried out and the results can be expected to be good.
