Pneumothorax within 30days after Surgery may not be a Real Recurrence

Purpose : There are many studies about the risk factors of recurrence of primary spontaneous pneumothorax after surgery (RAS). However, there has been limited research on the recurrence time interval after surgery (RT). Hence, this study was performed with an emphasis on the relationship between RT and RAS

Methods : We retrospectively reviewed all patients with RAS between January 2007 and March 2018. Patients were divided into two groups based on the time interval: the early recurrence group (RT≤30days) and late recurrence group (RT>30days). The patient baseline data, management method, and recurrence rate after RAS were compared between the two groups. Risk factors of recurrence rate after RAS were analyzed. Using the cutoff-point determination methods, the optimal cut-off point of RPOT was evaluated.

Results : A total of 78 cases (75 patients, bilateral 3 patients) were included in this study; 21 cases in the early recurrence group and 57 cases in the late recurrence group. Patient characteristics and operative method were not significantly different between the two groups. The recurrence rate after RAS in the late recurrence group (28.1%) was significantly higher than that in the early recurrence group (4.8%) (p = 0.031). The only significant risk factor of recurrence rate after RAS on multivariable analysis was late recurrence (p = 0.045). The optimal cutoff point was 30 days.

Conclusion : Based on this study, RT appears to be an important risk factor of recurrence after RAS. Nonetheless, future studies evaluating RT and RAS are warranted.
