Clinical Outcome of Pericardial Window Formation for Intractable Pericardial Effusion

Purpose : Although most of pericardial effusion can be managed by pericardiocentesis, some patients require surgical intervention such as pericardial window formation. We investigated the clinical outcome of pericardial window formation according to the presence of cancer.

Methods : Patients who underwent pericardial window formation between October 2011 and March 2019 was reviewed for the analysis. Exclusion criteria are age under 18, pericardiostomy without pericardio-pleural window, extensive pericardiectomy, and inadequate follow-up data. Primary outcome was survival after surgery.

Results : A total of 105 patients were enrolled for the study. The average age was 61±13.6 years old and there were 50 female patients (47.6%). The etiology included malignancy(41.9%), previous cardiac surgery(14.3%), idiopathic(33.3%) and others(10.5%). There were 43 patients(40.1%) who underwent surgery using VATs(Video-Assisted Thoracoscopic surgery), 61 patients(60.9%) underwent minithoracotomy approach, and one patient underwent subxyphoid approach. The average operation time for VATs was 51.4±20.2 minutes and for minithoracotomy was 54.5±27.8 minutes. There were 3 cases of recurrence(2.9%) after surgery. The estimated 1 year-survival rate was poorer in the cancer patients (29.5%) than in the non-cancer patients(91.8%).

Conclusion : Pericardial window formation is an excellent method for pericardial effusion with low recurrence rate. However, in patients with malignant effusion, it should be performed only selected patients due to poor late survival.
