Outcomes of tricuspid edge to edge repair with or without TAP

Purpose : Edge to edge repair (Alfieri technique) has been suggested for complex tricuspid valve pathologies. However, it is mainly considered as adjuvant or additional procedure. The aim of our study is to compare the outcomes of tricuspid edge to edge repair without TAP or with TAP.
Methods : A total of 256 patients who underwent tricuspid edge to edge repair between 2001.1 to 2019.12 were categorized into the two groups: without TAP (n=67) or with TAP (189). Inverse probability of treatment weighting (IPTW) analysis and propensity score-matching was used with 56 patients in each group to adjust for the baseline differences between the two groups.
Results : Early mortality and morbidities did not differ between two groups. IPTW-adjusted survival analysis did not reveal difference between two groups in overall survival, late TR>moderate, significant tricuspid stenosis (TTPG≥5mmHg) and tricuspid valve reoperations. These results were consistent in the matched analysis. Trans-tricuspid pressure gradient (TTPG) did not differ significantly between two groups at discharge and at last follow up.
Conclusion : The results of tricuspid edge to edge repair without TAP showed comparable with tricuspid edge to edge repair with TAP. Tricuspid edge to edge repair can be considered to perform without TAP, if the annulus was small or late tricuspid stenosis is suspected.
