The Impact of proximal anastomosis technique of saphenous vein graft on Mid-Term Clinical Outcomes after Off-Pump Coronary Artery Bypass Graft on single artery group (Aorta versus LIMA)

Purpose : This study was designed to evaluate the impact of the anastomosis technique of saphenous vein graft(SVG)
on mid-term clinical outcomes after off-pump coronary artery bypass graft on single artery group (OPCAB)

Methods : We reviewed 1569 patients who underwent isolated OPCAB between 2009 and 2015. Among them, 399 patients who received one arterial graft (LIMA) and SVG were enrolled in this study. The patients were divided into two groups according to the proximal anastomosis technique of saphenous vein graft: Proximal anastomosis to aorta group (Ao, n=282) and Composit Y to LIMA (Y, n=117). The end-points were overall survival and major adverse cardiac and cerebrovascular events (MACCEs). The mean follow-up duration was 5.1±2.2 years.

Results : The baseline characteristics were different between the groups. So the propensity score matching was done and 234 patients were enrolled. Kaplan-Meier analysis was done and the 10-year freedom from MACCEs rate revealed no significant difference between the groups (86.4±3.3% in Ao group vs. 86.2±3.4% in Y group, p =0.992). But the 10-year survival was significantly different between the groups (96.6±1.7% in Ao group vs. 90.3±2.8% in Y group, p=0.069). Cox-regression multivariate analysis was performed and the anastomosis technique was not associated with overall MACCEs (HR=0.911, 95% CI:0.500-1.659, p=0.761) but associated with survival rate (HR=2.463, 95% CI:1.026-5.915, p=0.044)

Conclusion : In a single-institution experience, the anastomosis technique of saphenous vein was not associate with improved mid-term MACCEs but associate with survival rate.
