Trends of Early Outcomes after Lung Transplantation

Purpose : In Korea, first lung transplantation (LT) was performed in 1996. The number of LTs gradually increased and a dramatic increase occurred in recent years. In this study, we report the annual results of LT performed in a single center in Korea.
Methods : All adult patients who received bilateral lung transplantation between January 2011 and June 2019 were reviewed. Preoperative patient status, operation status, postoperative results, post LT 3 months and 1 year survival rates were reviewed.
Results : Preoperative ventilator and ECMO applied rate increased in 2019, consisting of 90.9% and 81.8% accordingly. There was an increase in ischemic time. Postoperative bleeding rate showed a decreasing tendency after applying ECMO as operative support, occurring in 1 (9.9%) case needing operative treatment during 2019. Post-operative 72 hours PGD occurred most in 2017 (30.8%) but incident rates decreased. Post-operative ventilator duration decreased but the ICU stay and hospitalization period increased in recent years. Postoperative 3 months and 1 year survival rates were improved showing 100% and 81.8% accordingly in 2019. Preoperative status of LT patients worsened with more ECMO and ventilator being applied prior to LT and leads to longer admission. Ischemic time increased, but this did not affect the postoperative survival. Using ECMO reduced the rate of postoperative bleeding. Even with more severe patients, early outcomes of LT were not inferior in the recent years.
Conclusion : There has been an increase in number of LT performed during the recent years and the severity of the patients were also increased, but the postoperative LT results improved.
