Lymph node metastasis in patients with a small sized non-small cell lung cancer manifesting as radiological pure solid nodule.

Purpose : Small sized early NSCLC (≤ 2cm) are actively investigated by researcher for being dominant and possibility of reducing the surgical extent and/or systematic lymph node dissection. This study was to investigate the frequency of lymph node metastasis and oncologic outcomes of surgical resection in clinical N0 NSCLC with a size of 2 cm or less.

Methods : We retrospectively reviewed the medical records of the 1329 patients who underwent curative surgical resection with complete lymph node dissection for small sized clinical N0 NSCLC (≤ 2cm) in a single center from January 2003 to December 2017. Preoperative chest computed tomography was carefully reviewed by radiologist and thoracic surgeon for solidity judgement according to the size of its tumor (10mm or less, 10-15 mm, 15-20mm).

Results : The patients were divided pure solid (n=738) and mixed ground glass nodule (GGN, n=591). Demographics, sub-lobar resection rate, the number of dissected LN, LN metastasis rate and recurrence rate showed statistical differences between two groups (Table 1). LN metastasis rate showed no statistical difference depending the tumor size increase in mixed GGN (<10mm:2.3%, 10-15 mm,2.2%, 15-20mm:2.2%). However, it showed statistical significant difference in pure solid group (<10mm:2.7%, 10-15 mm,12.5%, 15-20mm:21.3%). In pure solid group, overall survival and recurrence-free survival are similar between lobectomy and sublobar resection (OS:p=0.28,RFS p=0.96)

Conclusion : A positive correlation between the tumor size and lymph node metastasis exists in pure solid NSCLC. Even small-sized lung cancer, especially more than 1 cm, should be evaluated for metastasis through lymph node dissection when possible.
