Effect of mediastinal lymph node dissection in pulmonary metastasectomy for colorectal cancer

Purpose : This study was performed to investigate whether lymphadenectomy may affect overall survival (OS) in patients undergoing pulmonary metastasectomy (PM), and to identify predicting factors of OS.
Methods : We retrospectively reviewed 275 patients who received the PM of colorectal cancer from January 2010 to December 2016. Mediastinal lymph node dissection (MLND) was defined as the resection of at least six mediastinal lymph nodes according to IASLC criteria (N1 ≥ 3 and N2 ≥ 3).
Results : The mean age was 60.9 ± 11.6 years and male was 165 (60%). The median numbers of metastatic nodule was 2 (range, 1-17), Numbers of patients who have solitary, two, and more than 3 were 130 (47.3%), 58 (21.5%), and 86 (31.3%), respectively. The mean size of the largest metastatic nodule was 1.5 ± 0.9 cm. MLND was done in 41 (14.9%) patients. There was no difference in 5‐year OS (MLND+ 55.8% vs LND− 55.4%; p=0.9) between the two groups. Nineteen nine (36.0%) patients have more than 5 (normal level) of preoperative carcinoembryonic antigen (CEA), and mediastinal lymph node involvement was found in 16 (5.8%). On multivariate analysis, preoperative CEA level (p<0.001), greater number of metastatic nodules (p=0.016), size of the largest metastatic nodule (p=0.005), and lymph node involvement (p=0.008) were negative prognostic factor of OS.
Conclusion : Mediastinal lymph node involvement, preoperative CEA level, higher metastatic nodule number, and size have a negative effect on survival whereas MLND in PM does not relate to the survival.
