Long-term outcomes of totally thoracoscopic ablation in patients with atrial fibrillation after failed catheter ablation

Purpose : Totally thoracoscopic ablation (TTA) has been proved to be effective in treating atrial fibrillation (AF). We previously reported that a presence of a catheter ablation (CA) history does not affect the outcomes of TTA. We compared the long-term outcomes of patients with a previous CA history (CA group) and those without a previous CA history (nCA group).
Methods : 366 patients who underwent TTA for the treatment of AF between February 2012 and January 2019, were retrospectively analyzed. Among those, 51 patients had a previous CA history and propensity score matching (1:3) was used to identify patients with similar baseline characteristics. All patients were followed up at 3, 6, and 12 months and annually thereafter with 24-holter monitoring.
Results : Covariates were well balanced between the nCA group and CA group. When it comes to postoperative complications such as stroke, bleeding, pacemaker insertion and pulmonary complications, there were no significant differences between the two groups. Freedom from AF recurrence at 6 year was 61.7 ± 7% in the nCA group and 45.8 ± 11% in the CA group (P=0.055). On the cox regression analysis, preoperative left atrial volume index (hazard ratio 1.043; P=0.001), BMI (hazard ratio 1.094; P=0.005) were associated with AF recurrence. Previous catheter ablation history was not a risk factor for AF recurrence (hazard ratio 1.744; P=0.114).
Conclusion : Long-term outcome of TTA after failed CA showed no statistical difference compared with the patients without a history of CA who underwent TTA. Preoperative LAVI and BMI still were strong predictors of AF recurrence.
