The prognostic value of neutrophil to lymphocyte ratio on in-hospital mortality in chest trauma patients: a propensity score matching study

Purpose : In general, higher neutrophil–lymphocyte ratio (NLR) has been suggested as an useful bio-marker of inflammation in many different type of diseases.
We investigated the prognostic value of NLR in admission period and analyzed its effect on in-hospital mortality of critically ill patients with chest trauma.


Methods : We retrospectively analyzed 2122 patients with chest trauma, admitted to tertiary trauma center from January 2014 to May 2019. After performing of propensity score matching with age and gender, logistic regression analysis was used to evaluate the relationship between initial NLR and in-hospital mortality. The predictive value of initial NLR on in-hospital mortality was evaluated with Kaplan–Meier survival and Receiver Operating Characteristics curve analysis.

Results : The hospital mortality rate was 11.8%. There were 244 matched pairs identified using propensity score matching, based on in-hospital mortality.
Initial NL ratio was statistically lower in matched survival patients, compared to matched non-survival patients (9.1±8.1 versus 4.7±5.2. p value<0.001), and the value was reversed after 3 days (9.6±7.6 versus 17.7±13.9. p-value=0.004). The cut‐off value for initial NLR and NLR after 3 days were 5.6 and 9.7, respectively. Logistic regression analysis demonstrated that initial NLR (<5.6) was independently associated with in-hospital mortality (odds ratio: 0.89, 95% confidential interval: 0.86-0.93, p-value<0.001)


Conclusion : We confirmed that the change of NLR of early hospital period is a prognostic predictor of survival in critically ill patients with chest trauma.
