Pattern and cause of the mortality during the first year after pneumonectomy for lung cancer

Purpose : Pneumonectomy is a high-risk operation with a mortality rate of 5-10 percent within 30 days and a mortality rate of about 20 percent within a year. To understand the pattern of the morbidity and mortality after pneumonectomy, we investigated unexpected re-admission and the cause of death within one year after surgery.

Methods : We retrospectively reviewed 337 patients who underwent curative pneumonectomy for non-small cell lung cancer between January 2009 and December 2018. The causes of death or re-admission were classified into 5 categories (oncologic, infectious, pulmonary, cardiac, or other), and the timing and incidence of complications were analyzed.

Results : Of the 337 patients, 164 (48.7%) patients experienced complications and 17 (5%) patients died during the hospital stay. All patients who died during hospitalization died from pneumonia or ARDS. Of the readmissions that occurred within one year after discharge, 80% occurred within 90 days, and the causes of readmission were in the order of pneumonia, bronchopleural fistula, and empyema. The 90-day mortality rate after surgery was 7.1%, and the 1-year mortality rate was 20.8%. All deaths within 90 days after surgery were due to non-oncologic causes. Of 46 patients who died from 91 days to 1 year after surgery, 17 patients died from non-oncologic causes.

Conclusion : All patients who died within 90 days after pneumonectomy died from non-oncologic causes, and even after 90 days, a significant number of patients died from non-oncologic causes. Efforts are needed to reduce non-oncologic deaths through patient education and close monitoring.
