Experience of microsurgical training with heterotopic abdominal heart transplantation in rat model

Purpose : Although various models of microsurgical training have been proposed, microsurgery is a technically challenging procedure. Here, we present the experience of microsurgical training using a rat heart transplantation (HTPL) model in candidate of cardiothoracic surgeon.

Methods : Sprague-Dawley rats weighing 250 to 600 g were used, and a total of 110 training was performed from September 2018 to August 2020. Trainee was graduates of foreign medical schools without microsurgical experience. After induction of cardiac arrest with application of cold cardioplegia and ice slush, the donor hearts were harvested. We performed the end-to-side anastomoses in two sites: ascending aorta (Donor) to abdominal aorta (Recipient), main pulmonary artery (MPA) (Donor) to inferior vena cava (Recipient). Continuous running suturing was performed with 8-0 polypropylene in all procedures under 3.5x magnified surgical loupe.

Results : The diameter of the anastomotic vessels varied between 2.5 – 3.0 mm. Although the first successful result was made on the 20th trial, we achieved continuous stable success in the last 30 experiment. The main reason of failure in our initial trials were bleeding from the anastomosis site in recipient and/or primary graft failure. The median time for aorta anastomosis was 20.7 minutes at the initial 80 experiments and it has reached to 15.4 minutes at the recent 30 trials. Respectively, the median time of MPA anastomosis also decreased from 25.4 minutes to 15.9 minutes at the last 30 trials.

Conclusion : Establishment of rat heterotopic HTPL model was the feasible technique for beginner of microsurgery experience.
