The effect of inferior pulmonary ligament division after right upper lobectomy

Purpose : The benefits of inferior pulmonary ligament (IPL) dissection during video-assisted thoracoscopic surgery (VATS) right upper lobectomy for early-stage lung cancer remains controversial. Hence, the purpose of this study is to evaluate the effect of IPL preservation by comparing the lung volume and bronchial tortuosity of residual lobes after right upper lobectomy.
Methods : The medical records of all patients who underwent VATS right upper lobectomy for early stage lung cancer between January 2016 and January 2019 at two institutions were evaluated. Patients were divided into two groups: the preservation (P) and division (D) groups. The pre- and post-surgery lung volumes and bronchial tortuosity were measured via curvature index (CI) on CT images for comparison (CI-1, from the carina to the entrance of RUL bronchus; CI-2, from the carina to the RML/RLL bifurcation area; CI-3, from the carina to the RML medial/lateral segment bifurcation area).
Results : There were 73 and 102 patients in Groups P and D, respectively. Patient characteristics – age, gender, body mass index, and smoking status, preoperative lung volume, and tortuosity were not significantly different between the two groups. Changes in pre- and post-operative lung volumes (residual lobes) and bronchial tortuosity (CI-1, CI-2, CI3) were not significantly different between the two groups.
Conclusion : The findings of this study showed that the division of IPLs during RUL may have no beneficial effect on lung volume and bronchial tortuosity.
