Surgical endoleak repair with FET in a patient received aortic arch repair with debranching and TEVAR

Purpose : Endoleak repair is an emerging issue in the thoracic endovascular repair(TEVAR) era. We present a case undergoing surgical endoleak repair with a frozen elephant trunk to treat for an endoleak after hybrid TEVAR aortic arch repair.


Methods : A 62 years old man with an aortic arch aneurysm had treated with debranching and TEVAR. The followed imaging exam showed an endoleak and progression of aneurysm size. Endoleak type was not determined in the imaging workup. Surgical repair was performed rather than repeat TEVAR.

Results : The intraoperative finding revealed type 1a Endoleak. Therefore, the endoleak was repaired with direct suture between the prior stent graft and the native aorta. During the circulatory arrest, the antegrade stent graft was re-insertion to cover the prior stent graft. The postoperative imaging exam confirmed disappear of endoleak.

Conclusion : Minimize conventional surgery extent added with a frozen elephant trunk is a good alternative procedure for the complicated redo-aortic arch cases.
