Perineural invasion is associated with postoperative intra-abdominal recurrences in non-small cell lung cancer

Purpose : Prognostic significance of perineural invasion (PNI) is well demonstrated and known to be associated with distant metastasis in some cancers; however, how PNI affects prognosis in non-small cell lung cancer (NSCLC) patients was not yet revealed. The purpose of our study was to investigate the risk and the pattern of postoperative recurrences associated with PNI in NSCLC.
Methods : We retrospectively review 2,094 patients who underwent surgical resection for NSCLC between January 2009 and December 2016. Using propensity scores, we made two matched cohorts presenting PNI, and not presenting PNI, and compared the risk and the pattern of postoperative recurrences between two groups. Additional risk analysis for postoperative recurrences was conducted using Cox proportional hazard regression model.
Results : Of all, 105 (5.3%) patients presented PNI. After propensity score matching, PNI (+) group showed more intra-abdominal recurrences than PNI (-) group (p = .045). In addition, PNI was independent risk factor for intra-abdominal recurrences In multivarable analysis (p=.007). In subcohort group presenting PNI, pStage III, and adenocarcinom subtypes were independent risk factor for intra-abdominal recurrences (P=.027. and .024 respectively). When PNI was present in pStage III adenocarcinoma, intra-abdominal recurrences was occurred in 55.6% of patients.
Conclusion : PNI is associated with postoperative intra-abdominal recurrences in NSCLC, especially in advanced adenocarcinoma. Our results could help to define the postoperative follow-up strategies for patients who underwent surgery for NSCLC presenting PNI.
