Minimally Invasive Approach of Aortic Bypass Grafting for Middle Aortic Syndrome

Purpose : Our team has striven to develop approaching techniques of aortic bypass for middle aortic syndrome(MAS), and here we report two cases of aortic bypass using less invasive approach than previously reported.
Methods : Two aortic bypasses for MAS were performed through minimal invasive approach and the early surgical outcomes have been evaluated at our clinic.
Results : A 40-year-old woman was presented with claudication and her computed tomography (CT) showed luminal narrowing of the abdominal aorta. The ascending aorta to left common iliac artery bypass was performed. A right lateral thoracotomy through the fourth intercostal space and a 10cm midline laparotomy were used for proximal and distal approach respectively. She has remained free of claudication at 6-month follow-up. The second case is a 51-year-old woman with a presumptive diagnosis of Takayasu’s arteritis. Her CT revealed diffuse narrowing of the aorta. The ascending to abdorminal aortic bypass performed with the same approach. At 3-month follow-up, her blood pressure is well controlled.
Conclusion : We report two successful cases of aortic bypass for MAS through a right lateral thoracotomy with a mini-laparotomy. This approach brought outstanding cosmetic results as well as satisfactory surgical outcomes.
