Incidental Diagnosis of Cor Triatriatum in an Adult with Mitral Regurgitation

Purpose : Cor Triatriatum, also known as a divided LA chamber, is a rare congenital anomaly. We reported a case of an adult patient with mitral regurgitation who was incidentally diagnosed as Cor Traitriatum.
Methods : A 51-year old male patient was examined for aggravated dyspnea on exertion. Transthoracic echocardiograms showed severe mitral regurgitation with Cor Triatriatum Sinister (transmembrane mean pressure gradient, 12.3 mmHg). Atrial fibrillation with controlled ventricular response was noted on EKG. Surgical treatment was indicated due to symptomatic severe MR with atrial fibrillation.
Results : Mitral valve replacement with a mechanical valve, tricuspid annuloplasty with an annuloplasty ring, maze operation with cryoablation, and resection of the anomalous membrane in the left atrium were performed. The membrane dividing the left atrium had a small opening and was partially calcified. He underwent an uneventful hospital course and was followed up for 7 months without any complication.
Conclusion : Cor Triatriatum has a wide range of clinical manifestations, depending on the degree of obstructive pathology and the presence of concomitant pathology. Surgical treatment is recommended in patients with associated symptoms.
