The association between enteral nutrition and prognosis in ECMO patients

Purpose : There are controversies in early enteral nutrition (EN) of patients with extracorporeal membrane oxygenation (ECMO) because of the concern for the gastrointestinal intolerance. We evaluated the usual clinical practice of the nutritional support in patient with ECMO and the importance of EN for the patient outcome.
Methods : ECMO patients from March 2017 to February 2020 and survived more than 48 hours were included for the study. The calorie and protein supply considering their requirements, the proportion of EN in total calorie achievement at ECMO day 7, and the time to EN from the ECMO running was evaluated.
Results : A total of 60 patients including 16 venovenous ECMO patients were finally enrolled. They were 59 ± 11 years, 42 were men and 32 patients were successfully weaned off ECMO and thirty were finally survived. EN was started at ECMO day 2 (1-3), and 77.6% of the patient achieved more than 80% of their calorie requirement within a week. Neither the calorie achievement (%) nor the proportion of EN in total calorie achievement at first week were different by patient survival (96.0 ± 41.2% versus 108.2 ± 39.9% p = 0.327; 53.8 ± 42.1% versus 34.0 ± 38.9%, p = 0.111). Only early EN started within 48 h from the ECMO significantly increased patient survival (HR 0.413, 95% Cl 0.174-0.984, p = 0.046).
Conclusion : Nutrition support was properly achieved within a week. Even EN does not become a main supply of the energy in ECMO patients, early EN can be recommended for better outcome.
