Effective Treatment of Secondary Pneumothorax by Packing Method

Purpose : Secondary pneumothorax is conservatively treated with chest tube drainage and pleurodesis in the elderly. However, surgical treatment may be required if air leakage persists. In most cases, surgery was performed to cut out the ruptured bullae. In this case, the packing method was performed using TachoSil and surgical glue.
Methods : A 77-year-old male visited the emergency room due to dyspnea. The patient recurred right pneumothorax again three days after discharge. Conservatively, we inserted chest tubes and performed pleurodesis. However, the air leakage lasted more than two weeks, and we decided on VATS operation.
Results : Previously, pleurodesis was performed and the visceral pleura was thickened overall. To find out the lesion, we performed air leakage test using water. Holes were observed in the interior of the RUL fissure surface, and there was a large amount of air leakage. The bare surfaces of the lung were exposed inside the hole. TachoSil and surgical glue were packed into the lesion, which was deep and difficult to wedge resection. There was no more air leakage from the lesion and the patient was discharged 7 days after surgery.
Conclusion : In secondary pneumothorax surgery, it is often difficult to find and cut out the air leakage lesion. We report a successful treatment by packing TachoSil and surgical glue.
