Case Report : Thoracoscopic Enucleation Of Neurilemmoma At The Thoracic Apex Of Two Patients

Purpose : Schwannoma is one of the most common primary tumor arising from the brachial plexus. Conventionally, supraclavicular incision or posterolateral thoracotomy is prefered by surgeons to remove the schwannoma because they provide good therapeutic results. Recently mini-open approach using thoracoscopy was also reported and operators keep on trying to find the way how operators can solve the tumor with more minimally invasive procedure. This case report can provide more minimally invasive way to remove the schwannoma at the thoracic apex with smaller incision and pain and saving rib without complications.

Methods : At October 2015 and January 2017, 2 patients underwent enucleation of neurilemmoma at the thoracic apex using a thoracoscopy with three small holes in the chest.

Results : The neurilemmoma was enucleated meticulously by freer and endoscissors. The patients have lived without any complications and recurrence so far. First patient’s operation time was 1 hour 52 minutes and estimated blood loss was scanty and chest tube was removed at post-operative day 2 and the patient was discharged at post-operative day 3. Second patient’s operation time was 1 hour 37 minutes and estimated blood loss was scanty and chest tube was removed at post-operative day 1 and the patient was discharged at post-operative day 2.

Conclusion : Thoracoscopic enucleation of neurilemmoma can provide alternative way to remove the neurilemmoma at the thoracic apex less invasively comparing with conventional posterolateral thoracotomy or supraclavicular incision. This method has advantages in aspects of pain, scar and shortened hospital-day without complications.
