The Correlation between Adipose Component in the LA Appendage and Recurrence of Atrial Fibrillation after Modified Cox-Maze III Operation from Human Tissue Pathologic Analysis

Purpose : The aim of study was to investigate the correlation between the recurrence of AF and adipose component of left atrial appendage (LAA) in AF who underwent modified Cox- Maze(CM) III operation.
Methods : We reviewed 165 patients’ LAA tissues pathology from which underwent modified CM III operation and analyzed the relation between adipose component (%, pathology slide section) and the recurrence of AF from 2009 to 2018. We followed up the patients` electrocardiogram in every 6 months after the operation and used receiver operating curve for gaining cut-off value of adipose component for AF recurrence.
Results : Of all, 128 of 165 patients (42 males, median age: 60 [interquartile range (IQR) : 34-81] years) were recovered to normal sinus rhythm (NSR). The mean follow up duration was 33.9 months (1- 121 months) and 30 patients (23.4 %) recurred AF and mean recurred duration was 26.8 months (4.4-114.4 months). The adipose tissue component in recurred population was significantly higher compared with non-recurred patients (16.3% vs 10.0%, p=0.007, respectively) and the meaningful cut-off value of adipose tissue component was 10 %. The AF free survival rate of high adipose group (≥10%) in 1, 5 years was 87.1%and 50.4% vs 96.7% and 76.8% in low adipose group(<10%), Log-Rank p=0.018, respectively)
Conclusion : High adipose component (≥10%) in LAA in the patients who underwent modified CM III operation showed higher recurrence rate of AF. Therefore careful checkup the lipid profile perioperatively and cautious follow up is important for patients with high adipose group.
