Clinical Outcomes of Thoracoscopic Surgery under Local Anesthesia for Primary Spontaneous Pneumothorax

Purpose : Video-assisted thoracic surgery (VATS) could be one of options to prevent recurrence of pneumothorax in case of patient with recurrent primary spontaneous pneumothorax (PSP). Since the technologies of anesthesia and surgery have been advanced, VATS under local anesthesia (LA) has widely been performed. Herein, postoperative clinical outcomes were compared to evaluate the feasibility and efficacy of VATS under LA compared with general anesthesia (GA) in patients with PSP.
Methods : 255 patients underwent wedge resection under VATS for PSP in our hospital from January 2014 to June 2019. Of them, 30 patients underwent operation under LA and the others underwent operation under GA. The surgical technique was relatively identical in both groups. All medical records were retrospectively reviewed
Results : The total operation time, post- chest tube drainage and total hospital days were significantly shorter in LA group, p=0.001, 0.017, 0.003, respectively. Although the visual analog scales (VAS) scores in immediate postoperative were not significant, VAS score in outpatient clinic was significantly lower in LA group than GA group (p=0.000). The incidence of postoperative recurrence after discharge in LA group (3.3%) was also lower than in GA group(18.7%) (p=0.001). In LA group, there was no case of conversion to intubation.
Conclusion : Our result showed relatively better clinical outcomes in VATS under LA with sedation than under GA in treatment of PSP. Therefore, LA with sedation could be one of good anesthetic options in VATS and could be applicable in surgical treatment of PSP.
