Perioperative Outcomes of Robotic Versus Single Port VATS Lung Resection

Purpose : We analyzed the perioperative outcomes of robotic and single port VATS surgery to evaluate the effect of changes in surgical techniques for lung resection.

Methods : From March 2016 to December 2018, prospectively, 87 patients who received lung surgery enrolled in our study. Forty patients underwent single port VATS lung surgery (Group A), and 47 patients received robotic surgery (Group B). We compared the patients’ characteristics and investigated perioperative outcomes.

Results : Two groups were not significantly different in patients characteristics except age (P<0.001). In group A, we performed lobectomy in 40, segmentectomy in 1, and sleeve lobectomy in 2 patients. In group B, we underwent forty-two lobectomies and five segmentectomies in lung malignancy. There was no conversion to thoracotomy and mortality in both groups. Operation-related morbidity was not significantly different (group A, n=9 vs. group B n=7, p=0.414). We removed a chest drain after 4.6 days in group A and 3.8 days in group B. The operation time was longer in robotic lung resection (150±46 min vs. 224±70, p<0.001) but hospital stay was longer in single port VATS (7.3±3.3 vs. 6.5±3.1, p=0.024). The medical cost was higher in the robot-assisted lung resection group (USD 8,029±209 vs. USD 15,054±206, p<0.001).

Conclusion : Robot-assisted lung surgery showed acceptable perioperative outcomes of morbidity and hospital stays compared to those of single port VATS lung surgery. However, the long operation time and high medical cost of the robotic surgery are still critical points to widespread robot-assisted lung surgery.
