Surgical Treatment for Right Atrial Appendage Aneurysm with Persistent Atrial Fibrillation

Purpose : Idiopathic right atrial appendage aneurysm (RAAA) is an uncommon cardiac anomaly characterized as a giant right atrium with its thin wall, in the absence of other causes. Treatment strategy of surgical correction for RAAA has not yet been instituted due to its rarity. We reported a case of RAAA to discuss appropriate indication and timing of surgical treatment for such disease.
Methods : A 56-year-old female patient presenting chest discomfort was referred to consider a surgical option for RAAA. Atrial fibrillation (AF) had not been shown on her initial electrocardiograms before two and a half years ago; however, RAAA has grown consistently and AF has occurred during follow-up period. Therefore, performing surgical corrections for RAAA and persistent AF was decided.
Results : A right atrial reductoplasty and maze operation using cryoablation were uneventfully performed via median sternotomy. The enlarged right atrial appendage was severely adhesive to the pericardium and intracardiac thrombi was not noted. After uneventful hospital course, she was regularly followed up for six months without any evidence of recurred AF.
Conclusion : It may be reasonable to perform surgical treatment for consistently growing RAAA with newly developed AF. Serial surveillance in patients with RAAA should be necessary to determine proper timing of surgical correction.
