Clinical Application of Vacuum Bell Therapy for Pectus Excavatum; finding Suitable Candidates.

Purpose : The aim of this study is to find appropriate candidates for applying vacuum bell therapy for pectus excavatum patients.
Methods : The medical records of pectus excavatum patients who have undergone vacuum bell therapy between August 2016 and February 2019 were reviewed retrospectively. Patients who have maintained their therapy for more than 10 months were included. The changes of Haller indices(HI) before and after vacuum bell application were calculated using chest Roentgenographies. Enrolled patients were classified into two groups according to the median value of changes in HI(0.45). Patient characteristics, treatment results including changes in sternum depression and complication rates were investigated.
Results : A total of fifty patients were included in this study. Group 1(Changes in HI less than 0.45) comprised 24 patients and Group 2(Changes in HI more than 0.45), 26 patients. The mean treatment periods were 17.4 months. The mean ages of Group 1 were significantly older than those of Group 2. (18.0±9.87 vs 13.4±5.11 respectively, p=0.043). Both HI measured before and 1 year after treatment of Group 1 were significantly lower than those of Group 2(3.0±0.37 vs 4.1±0.88 p<0.001 and 2.8±0.43 vs 3.2±0.62 p<0.001, respectively). The sternal depth(mm) measure before and after treatment of Group 1 were significantly lower than those of Group 2(2.3±0.82 vs 3.0±1.10 p=0.014 and 1.6±0.97 vs 2.2±0.64 p=0.009, respectively).
Conclusion : The application of vacuum bell therapy could be more available for patients with younger ages and severe chest depression. Patients who showed chest elevation less than 7mm for 1 year should be considered to undergo surgery.
