Extracardiac Rupture of a Sinus of Valsalva Aneurysm

Purpose : Sinus of Valsalva aneurysm (SVA) is a rare cardiac anomaly. Rupture of an SVA into the pericardial cavity is very rare with a reported frequency of only 1%.
Methods : A 52-year-old man was presented with abrupt onset of severe chest pain and dizziness. On physical examination the patient had a blood pressure 60/40mmHg with sinus tachycardia. A chest CT and TEE revealed a giant aneurysm of right valsalva sinus (70mm) with large amount of pericardial fluid and severe degree of aortic insufficiency. On the basis of these findings, the patient was scheduled for salvage operation. On the way to the operation room, he had a cardiac arrest but he was successfully resuscitated after prompt ECMO support. Sternotomy and pericardiotomy was done during ECMO support. The patch is sutured to the sinus defect, aneurysm was resected and oversewed. ECMO was removed in the operation room.
Results : He was discharged from the hospital on the 12 days without any complications and residual aortic insufficiency. At his last examination, nine months after his surgery, the patient was in a good physical condition.
Conclusion : Herein, we have reported a very rare case of rupture of an SVA into the pericardial cavity and underwent surgery successfully.
