Mode of Failures after Repair of Thoracoabdominal Aortic Aneurysm

Background : Thoracoabdominal aortic aneurysm is usually treated by replacement with vascular graft. With increasing number of long-term survivors, late-complications associated with the vascular graft gathers increasing clinical interest. The aim of this study is to survey the mode of complications and its frequency in patients who had undergone replacement of thoracoabdominal aorta.

Methods : Total 140 patients underwent replacement of thoracoabdominal aorta in Seoul National University Bundang Hospital, from November 2011 to July 2016. Among the 140 patients, there were 13 mortalities. Operation records, postoperative CT angiography, outpatient clinic records were reviewed for graft-associated complications for the 127 survival discharge cases

Results : Median follow-up duration was 29 months (IQR 11 – 71 months). Total 17 late mortalities occured. Mean overall survival was 105.70±3.56 months (C.I.95% 98.67 – 112.74 months). Total number of long term complications associated with graft was 8 (6.35%) cases. Among them, bleeding or pseudoaneurysm of graft anastomosis site were 2 (1.59%) and aneurysmal dilatation of anastomosis sites were 4 (3.17%). Graft infection occurred in 2 (1.59%) patients. Other than the area of operation, progression aortic aneurysm in remaining native aorta was combined in 8 patients (6.35%).

Conclusion : Replacement of thoracoabdominal aorta can cause various long-term complications. Cautious follow-up is necessary.
