Ideal Length of Proximal Resection Margin in Patients Who had Complete Resection Margin after Esophagectomy in Esophageal Squamous Cell Carcinoma

Background : The ideal length of proximal resection margin in esophagectomy is controversial. The aim of this study is to identify the risk of loco-regional recurrence according to the length of proximal margin.
Methods : Patients with esophagectomy for esophageal squamous cell carcinoma were included. Patient with positive resection margin, neoadjuvant therapy and incomplete medical record were excluded for survival analysis. A total of 138 patients from June 2003 to December 2013 were included.
Results : The median length of proximal margin was a 4.2 ± 2.6 cm. The loco-regional recurrence free survival was increased stepwise according to the length of proximal margin until 4cm. In patient with proximal margin of 4cm or greater, the loco-regional recurrence free survival showed inconsistency. In univariate analysis, patient with proximal margin of 4cm or greater had a better loco-regional recurrence free survival than who had proximal margin less than 4cm (p=0.005). In multivariate analysis, the T stage (p=0.004) and length of proximal margin (p=0.04) were risk factors for loco-regional recurrence.
Conclusion : The length of proximal margin is related to loco-regional recurrence. The proximal margin more than 4cm is recommended in esophagectomy for esophageal squamous cell carcinoma.
