Early Experience of Right Ventricular outflow Tract Reconstruction using Contegra Bovine Jugular Vein Conduit in Children

Background : The aim of this study is to review single center experience of right ventricular outflow tract (RVOT) reconstruction with Contegra valved conduit in pediatric patients.
Methods : From March 2016 to August 2016, 17 pediatric patients (10 females) underwent RVOT reconstruction with bovine jugular vein valved conduit (Contegra, Medtronic Inc., Minneapolis, MN, USA) as a primary Rastelli-type repair (n=5) or replacement of previously placed right ventricle to pulmonary artery conduit (n=12). Underlying diagnoses were pulmonary atresia with ventricular septal defect in 13, tetralogy of Fallot in 2 and Persistent truncus arteriosus in 2. Median age and body weight at operation was 29 months (range, 0 to 240 months) and 10.5 kg (range, 2.9 to 63kg), respectively. The size of Contegra were 12mm in 6 patients, 14mm in 6 patients, 16mm in 3 patients and 18mm in 2 patients. Median conduit size-to-body weight ratio was 2.0 (range, 0.29 to 4.18). 10 patients (10/17, 58.8%) underwent combined pulmonary artery angioplasty. Of them, Contegra conduit remnant was utilized in 5 patients.
Results : There was no early or late mortality, and no patient had conduit related complications. Postoperative echocardiography show good valve mobility, and median pressure gradient across the conduit, the right and the left pulmonary arteries were 9.0, 15.3, 14.5 mmHg, respectively.
Conclusion : The use of Contegra for RVOT reconstruction leads to satisfactory early outcome. Pulmonary artery angioplasty utilizing conduit remnant is technically feasible and cost-effective.
