Surgical Outcomes of Primary Malignant Cardiac Tumors

Background : Malignant cardiac tumors are rare and its management remains a poorly researched in clinical field. The purpose of this study is to review the experience of primary malignant cardiac tumors in single center.
Methods : Between January 1990 and May 2016, 37 patients with primary malignant cardiac tumors were identified. Of them, seven patients who underwent excisional biopsy (n=6) and O & C (n=1) were excluded. Among 30 patients (19 females, 63.3%), 26 patients received excision (complete excision in 9, incomplete excision in 17 patients) and four patients (13.3%) underwent heart transplantation (HTPL). Median age at operation was 22.5 years (range, 16 to 78 years).
Pre-operative adjuvant treatment was done in 3 patients; chemotherapy in 2 (6.7%), radiotherapy in 1 (3.3%). 19 patients received post-operative additional chemo-radiation therapy (16.7%), chemotherapy (40%) and radiotherapy (6.7%).
Results : The most common histological type and origin of tumor were angiosarcoma (33.3%) and right atrium (30.0%), respectively. The median survival was 12.8 months (range, 0 to 169 months); 13.2, 12.9 and 14.1 month in complete excision, incomplete excision and HTPL group, respectively. Local recurrence or distant metastasis was in 11 patients (36.7%) in spite of concurrent chemo-radiation or chemotherapy was done in 9 (81.8%) of them. The median duration of recurrence was 9.1 months (range, 0 to 140 months) from initial diagnosis.
Conclusion : Malignant cardiac tumors have poor prognosis without reference to type of surgical treatments. Preoperative and postoperative multi-modality treatment should be investigated to improve outcomes of these patients.
