Background : In Korea, durable LVAD is still rare performed because of high cost for device implantation. Thus, it is only performed for a critically ill patient who usually have extracorporeal life support.

Methods : We have performed Heartware HVAD implantation through minimally invasive incision in a patient who is on extracorporeal apico-aortic LVAD.

Results : The procedure was successful with moderate technical difficulty. The patient's recovery was complicated by MRSA septic shock, RV failure, aspiration pneumonia, ventilator dependency, severe cachexia, and swallowing difficulty. However, he went home after 242 days of hospitalization.

Conclusion : Minimally invasive LVAD implantation is a viable option for a patient with high risk of wound complication.
