Aortic Valve Replacement with Sutureless Valve in High-risk Patients

Background : Aortic valve stenosis (AS) is the most common valvular heart disease in the elderly population and if untreated, it is associated with poor outcomes. Nevertheless, 30–40% of patients with severe AS are denied the surgical procedure owing to high surgical risk for advanced age and multiple comorbidities. In this setting, transcatheter aortic valve implantation (TAVI) offers an alternative treatment option in high-risk patients. We performed aortic valve replacement in high-risk patients using sutureless bioprosthetic aortic valve

Methods : From July 2015 to July 2016, 4 patients with severe aortic stenosis underwent sutureless aortic valve replacement using Perceval S. Mean age was 84±2.8 years (100% octogenarian). Three patients were female. Mean Euroscore was 26±2.4 Mean ejection fraction was 37.4±12.3 %. Two patients had low ejection fraction (<30%), and one patient had myasthenia gravis. One patient had colon cancer

Results : There was no surgical mortality. Mean time for applying of ventilation in intensive care unit (ICU) was 4.75±1.3 hours, and all patients were transferred to general ward after one day of uneventful ICU admission. No valve thrombosis, migration, or structural valve deterioration occurred. New York Heart Association class improved at least 1 level in 100%. Total bypass time was 72.8±18.3 minutes and total aortic cross clamping time was 33.5±12.4 minutes. Mean systolic pressure gradient was 10.7±2.5 mm Hg after operation.


Conclusion : Aortic valve replacement using sutureless valve could offer an alternative treatment option in high-risk patients
