Efficacy of Single Port VATS Lobectomy in Lung Cancer (Compared with Triple Port VATS)

Background : In recent years, the number of single-port video-assiseted thoracoscopic surgery (VATS) for lobectomy of non-small cell lung cancer (NSCLC) patients is increasing. We performed this study to compare the feasibility and safety of single-port and triple-port VATS lobectomy.

Methods : We performed retrospective study on 73 patients with NSCLC who underwent VATS lobectomy from December 2011 to August 2016. There were 47 cases in triple-port group and the other 26 patients in single-port group.

Results : Operation time and intraoperative blood loss were similar in triple-port and single-port group (185.40±48.06 min vs 205.38±50.63 min, p=0.100 and 328.72±530.97 mL vs 314.61±513.10 mL, p=0.913). There was not morbidity and mortality case, and there were not significant differences in complications, total number of dissected lymph nodes. In single-port group, lymph nodes were dissected in more various mediastinal area (1.66±0.70 vs 1.23±0.51, p=0.004). Both group had one patients of bronchopleural fistula each. Excepts these two cases, in 71 patients, chest tube duration and postoperative hospital stay were shorter in single-port group (8.50±5.03 days vs 5.04±3.52 days, p=0.003 and 11.59±5.50 days vs 8.48±3.42 days, p=0.005). In single-port group, the rate of conversion to multiport was 11.54% (2 of 26). Rate of conversion to open thoracotomy in single port and triple port group was 3.85% and 6.38% (p=0.551).

Conclusion : In comparison with triple-port group, single-port VATS lobectomy showed faster recovery after lobectomy, and similar results in safety and efficacy. As more experience is accumulated, a large number of triple-port VATS lobectomy might be replaced by single-port VATS lobectomy.
