The Lifting Technique for Left Recurrent Laryngeal Nerve Lymph nodes Dissection in Esophageal Cancer

Background : This video demonstrates the ‘lifting’ technique for left recurrent laryngeal nerve lymph nodes dissection in esophageal cancer.

Methods : For the dissection of left recurrent laryngeal nerve lymph nodes, the esophagus is lifted upward. This technique can be used in either thoracoscopic esophagecotmy and robotic esophagectomy. In thoracoscopic esophagectomy, the loop is encircled around the esophagus, and loop is lifted. In robotic esophagectomy, the third arm with Prograsper can lift the esophagus. Under the lifting the esophagus, left recurrent laryngeal nerve and cardiac branches of sympathetic nerve can be easily found, and fat tissue and lymph nodes can be dissected en bloc manner with attachment to the esophagus.

Results : This technique is applied in consecutive 24 cases (2 thoracoscopic esophagectomy and 22 robotic esophagectomy) and left side vocal cord palsy was developed in 3 patients (3/24, 12.5%). The mean numbers of dissected left recurrent laryngeal nerve nodes were 5.79 ± 2.81.

Conclusion : The ‘lifting’ technique for left recurrent laryngeal nerve lymph nodes dissection in esophageal cancer is a safe and feasible technique.
