Fluid Analysis of the Early Postoperative Chest Tube Drainage for the Early Diagnosis of the Post-sternotomy Mediastinitis

Background : There is currently no reliable method for the early diagnosis of the post-sternotomy mediastinitis. We aimed to validate the usefulness of fluid analysis of the early postoperative chest tube drainage for it.

Methods : Since August 2015, we have performed fluid analysis of the chest tube drainage at postoperative day 3 in patients who had undergone sternotomy with the purpose of the early diagnosis of the postoperative mediastinitis. Cell count, glucose level, and pH were checked and fluid culture was performed simultaneously. Medical records of 100 patients in whom the fluid analysis was performed until August 2016 were reviewed retrospectively.

Results : Fifty-two men and 48 women were included (median age, 65 years). Patients were classified into group M+ (n=6) and group M- (n=94) based on the occurrence of the postoperative mediastinitis. Mann-Whitney U test revealed significant differences in the median values of white blood cell (WBC) count (13,646 vs 975, p < 0.001), glucose level (5 vs 92, p=0.007), and pH (7.087 vs 7.350, p=0.01) between groups. When the cut-off values for the early diagnosis of the postoperative mediastinitis were determined as WBC count ≥ 9,000, glucose level ≤ 40, and pH ≤ 7.2, sensitivity, specificity, positive predictive value and negative predictive value of each parameter were 83.3%, 95.7%, 55.6%, and 98.95; 83.3%, 92.6%, 41.7%, and 98.9%; 83.3%, 84.0%, 25.0%, and 98.8%, respectively.

Conclusion : Fluid analysis of the early postoperative mediastinal drainage is useful in the early diagnosis of the post-sternotomy mediastinits.
